A valley of shadows and darkness

Psalms 23 has a valley of shadows and darkness.

Psalms 23:4 Yea, though I walk through the valley of the shadow of death, I will fear no evil: for thou art with me; thy rod and thy staff they comfort me. [kjv]

From where comes the "valley"?
From where comes the "shadow"?

What is a "valley"? Let us look at the first few verses of Psalms 23 leading up to the "valley of the shadow of death". How is the word "valley" there? What might it mean?

The LXX has the "middle/midst of the shadow of death". The word "valley" is not present.
